If you’re listed as your child’s parent or guardian on their Medicare record, you can request the statement. No Medicare? No problem. You’ll need to fill out an Australian Immunisation Register (AIR) – Immunisation History Form to get one.

How can you get a copy of your child’s Immunisation History Statement?

Medicare Online via MyGov – Log in to MyGov, head to your Medicare account, and download it under “Immunisation History.”
Medicare Express Plus App – Open the app, go to "Immunisation History", and view/download your statement.
Call the Australian Immunisation Register (AIR) – Phone 1800 653 809 (Mon–Fri) to request a mailed copy.
